ARE YOU ELIGIBLE FOR A FREE NHS HEALTH CHECK?
You must be between the ages of 40 and 74 and be registered with a GP in Kent and Medway.
If you answer NO to all of the following questions, then you ARE eligible for an NHS Health Check and can book below.
Have you ever been diagnosed with the following:
- Coronary heart disease (including heart attack or angina)
- Heart failure
- High blood pressure (hypertension)
- Genetic high cholesterol (familial hypercholesterolaemia)
- Chronic kidney disease (stage 3-5)
- Diabetes (high blood glucose/sugars)
- Atrial fibrillation (irregular heart beat)
- Transient ischaemic attack (TIA) or mini stroke
- Stroke
- Peripheral arterial disease (PAD)
- Are you currently being prescribed statins for the purpose of lowering cholesterol?
- Have you had an NHS Health Check in the past 5 years? (This would include a finger prick cholesterol test)
- Have you had any other check through the NHS and been found to have a 20% or higher risk of developing cardiovascular disease over the next 10 years?
- Are you on the Palliative care register?
